    • The initial compensation for this position ranges from $73,469.40 - $90,358.09 per year.
    • Salary is dependent on commensurate experience above the minimum qualifications for the role and internal equity considerations.
    • 8% License Differential for LMFT, LCSW, LPCC, PsyD, and select Peer Certifications.*
    • 7.5% Bilingual Differential for qualified positions*
    • $1,500 hiring incentive payout after 30 days and 6 months!*

        • Employee engagement and advocacy opportunities to advance our justice, equity, diversity, and inclusion agenda.About Our ProgramUplift Wraparound provides wraparound services for children with behavioral issues that would otherwise need residential or a higher level of care.Work Schedule: Mon-Fri, 9-5pmJOB SUMMARY:Working independently in the family's home and a variety of community and/or treatment settings, Facilitator I coordinates services with an understanding of the complexity of each family while blending the clinical care of the youth with the facilitation of the life domain planning process.RESPONSIBILITIES AND DUTIES
          • Provides facilitation and coordination in the development of individualized services for youth and families.
          • Implements strength-based family-centered methodology into planning.
          • Understands interactions of complex systems in the context of a family's life.
          • Elicits, values, and incorporates divergent perspectives in action plans.
          • Participates with families as a complete unit with focus on individual strengths/concerns/needs.
          • Facilitates the development and refinement of effective safety plans.
          • Ensures the involvement of natural members with the Child and Family team.
          • Documents clearly and accurately.
          • Participates effectively in both internal and external multidisciplinary clinical teams.
          • Monitors effectiveness and efficiency of interventions with youth and families.QUALIFICATIONSTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.MINIMUM EDUCATION AND/OR EXPERIENCE
            • Master's Degree (M.S./M.A.) in related field.
            • Experience serving individuals with complex needs including SED youth/families.OTHER SPECIFIC REQUIREMENTS
              • Availability to participate in 24-hour on-call program.PREFERRED QUALIFICATIONS
                • MHRS and licensure eligibility.HIGHLY DESIRABLE QUALIFICATIONS
