Clinical Services Coordinator

Job Summary: Talent Software Services is in search of a Clinical Services Coordinator for a contract position that can be worked 100% remotely. The opportunity will be one year with a strong chance for a long-term extension.

Primary Responsibilities/Accountabilities:

Make 25 outreach calls per day to members to offer care management services.
Conduct at least three outreach calls and send one letter per member within a week.
Document each call attempt, send letters, and record call outcomes in the Care Connect system.
Close cases in the Care Connect system if the member is unreachable after three attempts.
Complete a short questionnaire with members reached via phone and offer care management services using a script.
Organize caseload to ensure each member is contacted at least three times before closing.
Use Excel spreadsheets and Care Connect system to track calls and outcomes.
Check other internal systems for member contact information.


Qualifications:

1 year working in a managed care environment or health insurance.
Excellent telephonic communication and customer service skills.
Ability to use and document in multiple computer systems efficiently.


Preferred:

Experience working with the Medi-Cal population.
Working knowledge of community resources.
